Patients' medication process is a complex multi-stage and multiprofessional process and an important part of nurses' daily work. In this study, medication-related adverse outcomes were analysed using medication-related incident reports, patients' records using the Global Trigger Tool method, and observations of medication administrations. The study demonstrated that medication-related adverse outcomes are common and three methods produced different information about these outcomes.
